Thyroid cancer, particularly papillary thyroid cancer, is often slow-growing and may never pose a significant risk to health. Despite this, many patients with low-risk cancers were historically subjected to aggressive treatments like total thyroidectomy, radioactive iodine therapy (RAI), and lifelong hormone replacement.

“I’ve seen firsthand how aggressive treatments, although lifesaving for high-risk cases, have been unnecessarily applied to patients with low-risk cancers,” explains Dr. Sandeep Nayak. “These patients can experience complications such as voice changes, low calcium levels, and dependence on medication, all of which could have been avoided with more personalized care.”

Today, with better imaging technology and a more nuanced understanding of thyroid cancer, doctors can now differentiate between high-risk and low-risk cases and adjust treatment plans accordingly. This evolution in care has led to active surveillance becoming a preferred approach for many low-risk patients. Instead of immediate surgery, these patients are monitored regularly through ultrasounds and check-ups to ensure the cancer is not growing or spreading.

The key focus in modern thyroid cancer care is personalized treatment. For low-risk cases, lobectomy—the removal of only the affected lobe of the thyroid—has replaced total thyroidectomy in many cases. This approach reduces complications and eliminates the need for lifelong thyroid hormone replacement therapy.

“When a patient is diagnosed with low-risk thyroid cancer, the first question should always be, ‘Do we really need to remove the entire thyroid?’” Dr. Nayak says. “If the cancer is confined to one lobe and is small enough, removing just that lobe is often enough to eliminate the cancer and prevent recurrence.”

Moreover, radioactive iodine therapy (RAI), once considered routine, is now often avoided for low-risk cancers. “We’ve found that for many low-risk thyroid cancers, RAI is unnecessary,” Dr. Nayak explains. “There are far fewer side effects and complications when we skip this treatment, especially for patients whose cancer isn’t aggressive.”

Minimizing treatment isn’t about doing less—it’s about providing the right treatment at the right time. Dr. Nayak’s approach focuses on enhancing patient well-being while avoiding unnecessary risks.

“Each treatment option comes with risks, and while surgery and RAI are highly effective, they aren’t always necessary,” Dr. Nayak explains. “When we minimize treatment for low-risk cancers, we’re helping patients maintain a better quality of life with fewer disruptions to their daily routine.”

Avoiding unnecessary risks is crucial. Surgical procedures, like total thyroidectomy, and treatments like RAI can have significant risks. By tailoring treatments, unnecessary interventions can be avoided, reducing the likelihood of complications.
